We provide services to Medicaid-qualified individuals who meet the following criteria:
Must be 21 years of age or older
Must be eligible for Medicaid
Must meet medical necessity regulations
Turquoise Care Program
As part of the Turquoise Care program, each member selects a Managed Care Organization (MCO) that provides access to care coordination services. These services include creating an individual plan of care tailored to each member’s needs. The Health Risk Assessment (HRA) is conducted via telephone, ensuring every member receives the attention and support necessary to thrive at home.
